For Patients

Thank you for allowing CarePoint to provide services for you during your recent visit to the emergency department or hospital. It is our privilege to be a part of the care for you and your family.

While we would like to be able to help you after discharge, please know that we are not able to order new medications or refills after you are released. Items like oxygen, prescription refills, and home health care should be taken care of by your primary care physician. We do not have clinics or primary care physicians who are able to take care of your needs after your departure from the hospital or emergency department.

Post-Discharge Assistance

Medication Refills

Following your discharge, all prescription refills must be ordered and monitored by your primary care physician, or the specialist who follows your care if applicable. CarePoint Emergency Medicine physicians are unable to refill medications after you are discharged.  If your prescriptions require prior authorization, please contact your primary care physician.

Follow-up Care

Please contact the office of your primary care physician for follow up care if you were seen by one of our hospitalists or emergency medicine physicians. We cannot schedule appointments for our providers to see patients, as we are only able to provide care to patients that are in the emergency department or those who are hospitalized. If you have an emergency, please dial 911.
